About Dignity Hospice

Our Mission

Hospice is a specialized type of health care that focuses on providing comfort measures to patients with a terminal diagnosis. Rather than seeking curative treatments, at Dignity Hospice, we take a more holistic approach which focuses on the comfort of our patients, and the emotional well-being of their loved ones as the end of life journey is completed. We have a team of specialized clinicians from a variety of disciplines to ensure that the physical, emotional, and spiritual needs of our patients and their loved ones are met. We accomplish this by tailoring an individual plan of care for each and every patient who entrusts us with his or her care.

Cancer is a disease that takes place when there is an uncontrolled division of abnormal cells in a part of the body.

  • Palliative Performance Score of 70% or less
  • Decline regardless of therapy
  • Metastatic cancer
  • No longer seeking aggressive treatment

Neurological disorders include: Dementia, Huntington’s disease, MS, ALS, Parkinson’s, Alzheimer’s, and others. 

  • Several hospitalizations, trips to the emergency room, and visits to the doctor
  • Swallowing becomes a challenge/ choking on liquids or food
  • Unable to sit without assistance
  • Use of wheelchair or walker
  • Urinary/fecal incontinence
  • Unable to make facial expression

Liver disorders can be inherited and/or caused by damaging factors to the liver such as, auto-immune disorders, obesity, alcohol, and viruses.

  • Compromised daily activity
  • Hepatic encephalopathy
  • Serum albumin <2.5 gm/dl
  • Peritonitis
  • Recurrent variceal hemorrhage
  • Ascites
  • Malnutrition
  • Muscle wasting
  • INR > 1.5
  • Asterixis
  • Waiting for transplant, but if patient is procured they are no longer eligible

A stroke takes place when there is an interruption of blood supply to the brain.

  • Palliative performance score of 40% or less
  • Assistance required to complete daily activities
  • Bed/chair bound
  • Functional status is impaired
  • Orientation status changes
  • Progressive weight loss due to insufficient fluid/food intake

Kidney failure is the last stage of chronic kidney disease. This means the kidneys have ceased to work well enough for survival without transplant or dialysis.

  • Creatinine clearance of <10cc/min and serum creatinine >8.0mg/dl
  • Choosing not to have dialysis
  • Untreatable hyperkalemia
  • Albumin <3.5 gm/dl
  • Platelet count <25,000
  • Hepatorenal syndrome
  • Anorexia

Heart failure takes place when the heart is unsuccessful in properly pumping blood throughout the entire body. 

  • Ejection fraction <20%
  • Chest pain
  • Weight loss
  • Appetite changes
  • Endurance decline
  • Impaired rest
  • Little response to diuretics and vasodilators
  • Limitations to daily activities/mobility
  • Inadequate blood supply to heart

COPD involves difficulty/discomfort in breathing with constriction of the airways.

  • Frequent hopsptilations with plumonary infections/respiratory failure
  • Tightness of the chest
  • Limited daily activity
  • Appetite changes/progressive weightloss
  • Impaired rest
  • Reqiurement of oxygen most times
  • Limited mobility
  • Requirement of inhalers and/or breathing tre
